to my credit card number as listed above and to request a credit allowance
for long distance telemanagement services in the amount up to the figure(s)

every 28 days only if my usage is at least $20 for that four-week period.

details to dispute any charges and that all disputes must be in writing

credit card upon request and will pay any and all outstanding balances
